Director of Discharge Planning / Social Services

Louisa Health & Rehabilitation Center in Louisa, Virginia is looking for an experienced Director of Discharge Planning and Social Services to help residents and families prepare for their transition home. You will directly coordinate the patient's discharge including supplies, transportation, and other services to ensure a safe transition. Your day-to-day

  • Research, identify, and arrange external services for residents such as Chaplaincy, Advocates, Responsible Parties, and optimal care environments.

  • Directs timely and efficient five-day discharge planning meetings for all newly admitted residents.
  • Assesses patient's support systems and home situation to assist in decision-making regarding discharge plan and needs.
  • Facilitates effective bed management by strategic evaluation and internal arrangement to maximize availability of transitional and extended care services.
  • Meet with and assist residents with social issues such as conflict resolution, problem-solving, and care coordination.
  • Conduct customer satisfaction surveys.

 

Qualified candidates will have a 4-year degree in Social Work, Psychology, Sociology, Education, Counseling, Criminal Justice, or other human services related degree and at least 1 year of Social Work experience in a healthcare setting.

The Activities Assistant assists in the planning, conducting, and evaluating of scheduled activities within a very successful Activities Department. Additional job duties include leading group activities, maintaining attendance records, collecting oral histories, and completing patient assessments, progress notes, and care plans as directed by the Activities Director.

 

Minimum Requirement: An associate degreed (2 years) individual who qualifies as an Activities professional via (1) two years of experience in a social or recreational program within the last five years one of which was full-time in a therapeutic activities program, (2) certification by the National Council for Activity Professionals, (3) completion of a state-approved activities training course, or (4) immediately upon hire enrollment in a state-approved activities training course.

 

Preferred Requirement: A four-year college degreed individual who is qualified as a Therapeutic Recreation Specialist or Activities Professional.

RN/LPN Unit Manager

Louisa Health & Rehabilitation Center has an exciting opportunity for a Unit Manager, RN/LPN in our 90 bed skilled rehab center in Louisa.

 

The Unit Manager will oversee clinical operations of the unit and is responsible for staff supervision of nurses and CNAs, as well as interfacing with physicians, QI reporting and follow-up, and providing clinical expertise to staff as needed. This unit provides care for our most clinically acute patients and residents. Typical hours are Monday through Friday day shift, with some weekend and on-call duties.

 

Requirements:

•Current license by the Virginia Board of Nursing to practice as a Registered Nurse.
•Minimum three to five years of experience as an RN/LPN, preferably in long term care.
•Comprehensive knowledge of federal and state regulations as they relate to long term care.
•Nursing Management experience preferred.
•BSN preferred.
